Terms and conditions

The content on www.chloe-nutrition.com is intended for general use. The information contained on this site is intended to improve the health and well-being of customers. It is not intended to be a substitute for professional medical advice, diagnosis or treatment. If you have any questions regarding your health, please consult a healthcare professional.

Terms of Engagement

Both Chloé in her capacity as a Registered Nutritional Therapist, and her new client, will sign a Terms of Engagement form at the start of their Initial Consultation.

Consent

A detailed Consent Form will also be provided by Chloé to her client at the start of their Initial Consultation to enable her clients to choose how their data is used by Chloé Nutrition. 

Confidentiality

It is confidential that you are a client of Chloé Nutrition, as well as what you discuss in your sessions. There are however, some limits and exceptions to confidentiality.

If your consultant has serious concerns about your safety or the safety of another person normal confidentiality may be lifted. You will be asked to provide your GP’s contact details, as someone they may contact.

Please refer to our Privacy Policy for our full data protection policy which details which information we hold about you and how we use it.

Pre-consultation Questionnaires

These forms will be emailed to you and will need to be completed and returned via email in order for me to properly prepare for your first consultation.

 

Payments

For all our consultation services you are required to pay for your appointment or package via our website in advance.

  • Single appointments must be paid at the point of booking. You will receive an invoice at the time of booking the initial consultation.

  • Packages must be paid for upfront. Sessions do not have to be booked at the same time and can be booked separately on the 'Book online' page.

Please note that additional charges apply for any recommended tests and supplements. 

Cancellation Policy

  • single consultations

If an appointment is cancelled with less than 48 hours notice, a 50% fee will be incurred.

If an appointment is not attended, a 100% fee will be charged or one session will be deducted from package. 

  • packages

In the case of advance payments for services, you have the right to cancel this Agreement within 14 days of the purchase without penalty (‘Cooling-off Period’). If you wish to terminate the agreement within the 14-day Cooling-off Period, you must do so in writing to chloe@chloe-nutrition.com.  The nutritional therapist will refund any payments paid prior to cancelling this Agreement. If the services have commenced during the 14-day Cooling-off Period, the nutritional therapist will refund any payments due to you for the remaining sessions on the package.  The refund will be calculated based on the fee of a single consultation. The refund payment will be made within 14 days of cancellation.

If you pay for a package deal and wish to cancel your follow up appointments after the first session, the nutritional therapist will refund any payments due to you for the remaining sessions on thee package.  The refund will be calculated based on the fee of a single consultation. The refund payment will be made within 14 days of cancellation.

If follow up appointments for a package deal are not used within the time limit,  no refunds will be given as it will be assumed you no longer require the sessions and your contract will be terminated.

Time limits for packages: 3 months for the 'New client' package, 6 months for the '2 Follow-ups' and '3 Follow-ups' packages.

Cancellation of an Appointment by Chloé Nutrition

If an appointment is cancelled with less than 24 hours notice by Chloé Nutrition, you will be given a 50% reduction in the cost of your next appointment.

Clients Under the Age of 18

Children are welcome to attend the Chloé Nutrition clinic. They will need to be accompanied by a parent or legal guardian, who will also need to provide written consent during the Initial Consultation.

We request that our clients note the following:

  • The degree of benefit obtainable from Nutritional Therapy may vary between clients with similar health problems and following a similar Nutritional Therapy programme.

  • Nutritional advice will be tailored to support health conditions and/or health concerns identified and agreed between both parties.

  • Nutritional therapists are not permitted to diagnose, or claim to treat, medical conditions.

  • Nutritional advice is not a substitute for professional medical advice and/or treatment.

  • This document only covers the practice of Nutritional Therapy within this consultation, and your practitioner will make it clear if he or she intends to step outside this boundary.

 

The Client understands and agrees to the following:

  • I am responsible for contacting my GP about any health concerns.

  • I give permission for the nutritional therapist to contact my GP regarding any agreed aspects of my case.

  • If I am receiving treatment from my GP, or any other medical provider, I should tell him/her about any nutritional strategy provided by a nutritional therapist. This is necessary because of any possible reaction between medication and the nutritional programme.

  • It is important that I tell my nutritional therapist about any medical diagnosis, medication, herbal medicine, or food supplements, I am taking as this may affect the nutritional programme.

  • If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact my nutritional therapist promptly for clarification.

  • I must contact my nutritional therapist should I wish to continue any specified supplement programme for longer than the original agreed period, to avoid any potential adverse reactions.

  • I am advised to report any concerns about Nutritional Therapy promptly to my nutritional therapist for discussion and action.

  • Recording consultations using any form of electronic media is not allowed without the written permission of both the Nutritional Therapist and the Client.
